If you submit the form at a branch office, an employee of ours will also verify your identity.
If you have an electronic signature, you can also submit a request by email to dpo@otpbanka.si.
However, if you do not have an electronic signature, print the application, complete it and sign it where necessary, and send it by mail to the bank's address (OTP banka d.d., Slovenska cesta 58, 1000 Ljubljana, with the note "for DPO") or send it by email to dpo@otpbanka.si.
OTP banka reserves the right to contact you further during the resolution of your claim in order to verify or obtain additional information from you.
Your request will be handled by the Data Protection Officer, who will collect the requested data and prepare a reply for you. We will reply to you without undue delay and no later than one month from the receipt of your request. For complex or multiple claims, this term may be extended by two months, of which we will notify you.
